We can facilitate short-term stays at our hospice in Peterborough, where our expert multidisciplinary team can manage complex symptoms, offer emotional support, and provide specialist end-of-life care.

Thanks to funding from Cambridgeshire and Peterborough Integrated Care Board (ICB), we also offer nurse-led care for patients who don’t wish to die at home, but don't require acute care or specialist medical intervention.

This allows us to provide inpatient care for people who would not usually meet specialist referral criteria, while helping to relieve pressure in hospitals and the community.

Hospice at Home referrals

Our service is available seven days a week, and we are happy to take referrals from healthcare professionals at any time. Sue Ryder Thorpe Hall Hospice at Home service is commissioned to cover patients whose GP have a Peterborough postcode.

To refer a patient for Hospice at Home services, please complete the Cambridgeshire and Peterborough Clinical Commissioning Group’s Continuing Health Care Fast Track referral form and the associated NHS consent form, both of which can be found below.

Please note, that this referral form is only to be completed by healthcare professionals. If you are a patient or family living in Peterborough wanting to refer to the Sue Ryder Thorpe Hall Hospice at Home service, please speak to your lead healthcare professional or GP.

If you wish to make a Hospice at Home referral for patients under the care of a GP in the city of Cambridge, Fenland, East and South Cambridgeshire or Huntingdonshire please refer to Arthur Rank Hospice Charity.
