Support for young people
Sue Ryder Thorpe Hall Hospice provides care to young adults from the age of 18 years who live in and around Peterborough living with complex or life-limiting conditions.
To support young people aged 14-18 who are beginning to think about moving from children to adult hospice care services, we offer a dedicated transition programme to help welcome you to our hospice and services.
A warm welcome
Through our transition programme we do all we can to help young people adjust to adult hospice care, by offering a warm welcome, answering any questions you may have and making you feel more comfortable and confident about accessing our services which are new to you.
Our specialist team of doctors, Sue Ryder Nurses, physiotherapists, occupational and complementary therapists and family support team all work together to support young people, and their families, to make sure they can live the best lives they possibly can.
What happens when you are referred to us?
When a young person is referred to our service through the transition service, we work in partnership with the joint Transition Coordinator with East Anglia’s Children’s Hospices (EACH) to offer a range of support and services, including:
- open days
- tours
- Young Person Transition Coordinator support
- strong links with EACH to support transition of care.

Making a referral to Sue Ryder Thorpe Hall Hospice
Young people can be referred to Sue Ryder Thorpe Hall Hospice by their lead healthcare professional. Healthcare professionals can access our referral form through our Thorpe Hall referrals page.
